If you are a new patient coming to visit us for the first time, here are some essential information you need to know.

Insurances accepted

We accept many major insurance plans, including Medicaid. If you want to verify if your Insurance plan is accepted, please call us at 407-518-7700 during our business hours, or contact us online after hours.

Your First Visit

Save Time! Download Forms and fill them out before you visit the clinic.

Please bring the following items when visiting our office:

1. Photo Identification
2. All Insurance Cards
3. Your co-pay/deductible (if applicable)

Payment Options

Your co-pay is due at the time of your visit. We accept cash, Visa,  Master, Discover & American Express cards.

Apppointments and Cancellation

An appointment is not need, but recommended. It is a good idea to call before your visit to make sure that a Doctor is in. If you prefer to see a female Doctor, please call and make sure Dr. Aisha is in, or make an appointment ahead of time. 

If you need to cancel an appointment, please call us as soon as possible.
